Cascade Functional Medicine, Aesthetics, and Aviation, formerly known as Cascade Family Medicine, is a family-owned, family-centered practice dedicated to providing personalized care in a welcoming, smaller-clinic setting.

The practice was founded in 1988 by Lawrence E. Foltz, D.O., FAAFP, and his wife Erma, a registered nurse. Born and raised in Portland, Oregon, Dr. Foltz returned to the Northwest after completing his medical training and became one of the first osteopathic physicians to practice in Vancouver, Washington. They established Cascade Family Medicine to deliver thoughtful, individualized care rooted in osteopathic principles.

In 2008, Dr. Foltz’s youngest daughter, Sherilyn Foltz-Cook, D.O., MSCP, joined the practice, continuing the family legacy of comprehensive, patient-focused care. She is board-certified in Family Medicine with additional expertise in functional, preventive, aesthetic, and menopause care, and founded Cascade Aesthetics upon joining the clinic. Together, our physicians are committed to promoting healthy choices, educating patients, and creating a comfortable, family-friendly environment for individuals and families at every stage of life.
