Our clinic evaluates more pilots than any other in the Northwest Mountain Region. We understand the demands of a pilot’s schedule and strive to provide timely appointments for your exam. Our experienced team is well-versed in FAA medical certification requirements and will guide you efficiently through the clearance process.

We have two Senior Airman Medical Examiners (AMEs) who perform FAA flight physicals for Class I, II, III, and Air Traffic Controllers: Dr. Lawrence Foltz and Dr. Sherilyn Foltz-Cook.

How to apply for a medical certificate

  1. Login to Medxpress.faa.gov

  2. Complete the 8500-8 application and submit

  3. Save the confirmation number

  4. Call our office to schedule

What to bring to your appointment

  1. Photo ID

  2. Confirmation number

  3. Any medical documents (When required. See below). If unsure what medical documents are required, please call our office or look under the FAA AME GUIDE.

Aviation Exams

Color Vision

FAA requires a new, one-time, computer-based color vision screening for first-time pilots to ensure they can distinguish aviation signal lights. Approved tests are the Rabin Cone Contrast Test, Waggoner Computerized Color Vision Test, or City Occupational Colour Assessment & Diagnosis (CAD)

Doctor Consult

FAA doctor consultations are conducted by FAA-designated Aviation Medical Examiners (AMEs) to assess pilot fitness for flight. You must complete a MedXPress application and, if you have health conditions, bring relevant medical records to your appointment.

Nurse Consult

FAA medical consultationshelp pilots navigate certification requirements, particularly for complex health conditions, by providing expert guidance before, during, or after the MedXpress process

The following are some common medical conditions that will require you to bring documentation.
